Hormonal birth control is one of the most widely used medications in the world — and one of the least discussed in terms of its systemic effects. For many women, it works without obvious problems. For a significant number of others, it comes with side effects that range from inconvenient to genuinely life-altering, and that are often dismissed or minimized by prescribers.
This is not an anti-birth-control article. Contraception is a personal decision, and understanding the full picture — the benefits and the potential costs — is what allows you to make an informed choice. How Hormonal Birth Control Works (Briefly)
All hormonal contraceptive methods — the combined pill, progestin-only mini pill, patch, vaginal ring, hormonal IUD (Mirena, Kyleena, Liletta), implant (Nexplanon), and injectable (Depo-Provera) — work by delivering synthetic hormones that suppress or override the body's natural hormonal cycles.
The combined pill, patch, and ring contain synthetic estrogen plus a progestin (synthetic progesterone). The mini pill, hormonal IUD, implant, and injectable contain progestin only. The primary mechanism is suppressing ovulation by disrupting the hypothalamic-pituitary-ovarian (HPO) axis — the hormonal signaling chain between the brain and ovaries. Without the brain sending the right signals, the ovaries do not mature follicles or release eggs.
Because all of these hormonal axes are deeply interconnected — the HPO axis shares the hypothalamus and pituitary with the adrenal and thyroid axes — disrupting one inevitably affects the others. This is where many side effects originate.
Mood, Depression, and the Danish Cohort Study
The association between hormonal birth control and mood changes has long been dismissed as anecdotal. Then, in 2016, a landmark Danish cohort study published in JAMA Psychiatry followed over one million women for 13 years. The findings were striking: women using hormonal contraception had a significantly higher rate of first diagnosis of depression and first use of antidepressants compared to non-users. The risk was highest in adolescents and with certain progestins, and was present across all methods tested — pills, patches, rings, and hormonal IUDs.
The mechanism is multifactorial. Hormonal birth control depletes B vitamins (particularly B6, which is essential for serotonin synthesis), disrupts the HPA axis and cortisol regulation, and alters the gut microbiome — all of which directly affect neurotransmitter production and mood. Some progestins also bind to neurosteroid receptors in the brain, with effects on mood and anxiety that are not yet fully characterized.
Libido and SHBG
Low libido is one of the most commonly reported but least acknowledged side effects of hormonal birth control — particularly the combined pill. The mechanism is direct: the synthetic estrogen in combined methods dramatically increases sex hormone-binding globulin (SHBG), a protein that binds and inactivates sex hormones in the bloodstream.
Elevated SHBG means less free testosterone available to tissues — and testosterone is the primary driver of libido in women. Studies have found that women on the pill have SHBG levels up to four times higher than non-users. Critically, some research suggests that SHBG levels may remain elevated for months or even years after stopping the pill, which is one reason many women notice pill and libido issues that persist after coming off.
The Gut Microbiome
The gut microbiome — the community of trillions of microorganisms in your digestive tract — is profoundly affected by hormonal contraceptives. Research suggests that hormonal birth control, particularly the pill, alters the composition and diversity of gut bacteria, reduces populations of beneficial species, and increases intestinal permeability (leaky gut).
This matters for several reasons. Gut bacteria help regulate estrogen recycling via an enzyme cluster called the estrobolome. A disrupted gut microbiome means disrupted estrogen metabolism. Poor gut health also affects absorption of every nutrient the pill depletes — compounding deficiencies. And the gut-brain axis means that gut microbiome disruption directly contributes to the mood changes described above.
Autoimmune conditions including Crohn's disease, ulcerative colitis, lupus, and multiple sclerosis have all been linked to oral contraceptive use — likely through the leaky gut and inflammatory pathway.
Nutrient Depletion
Hormonal birth control is one of the most significant causes of micronutrient depletion in otherwise healthy women. The most important nutrients depleted include:
- B vitamins — B2, B6, B9 (folate), and B12: essential for energy, neurotransmitter production, DNA synthesis, and reproductive health
- Magnesium — involved in hundreds of enzymatic reactions including stress response, sleep, and hormone production
- Zinc — critical for immune function, ovarian follicle development, and skin health
- Selenium — needed for thyroid hormone conversion and antioxidant defense
- Vitamin C and E — antioxidants essential for reproductive health and immune function
These deficiencies do not disappear the moment you stop taking hormonal birth control. Rebuilding nutrient stores takes months of intentional dietary and supplementation effort — which is why supporting your body nutritionally while on hormonal contraception, and especially while coming off, is so important.
Acne — The Paradox
Hormonal birth control is frequently prescribed specifically to treat acne, and for many women, it does clear skin — at least while they are on it. But the story is more complicated. Some progestins (particularly those with higher androgen indexes, like levonorgestrel and norethindrone) can actually worsen acne by binding to androgen receptors. And for women who developed acne due to underlying insulin resistance, gut dysbiosis, or thyroid dysfunction, the pill suppresses the symptom without addressing the root cause — meaning acne often returns, sometimes worse, when the method is stopped.
Understanding which progestin is in your contraceptive method matters if acne is a concern. Lower androgen-index progestins (like desogestrel and drospirenone) are less likely to cause acne. But even these do not address root causes.
Weight Changes
The evidence on hormonal birth control and weight gain is genuinely mixed — large randomized trials have not consistently shown significant average weight gain. However, individual responses vary substantially, and some mechanisms are plausible:
- Some progestins have glucocorticoid activity, meaning they can promote fluid retention and appetite
- The depletion of B vitamins and magnesium affects energy metabolism and insulin sensitivity
- Elevated SHBG reducing free testosterone may reduce lean muscle mass over time
- Disrupted cortisol patterns (via HPA axis interference) can promote abdominal fat storage
The Depo-Provera injection is the method most consistently associated with weight gain in clinical studies, and this effect can persist for a significant time after the last injection.
Headaches and Migraines
For women who already experience migraines, the relationship with hormonal contraceptives is nuanced and important from a safety perspective. The combined pill (containing estrogen) can trigger or worsen migraines in some women — and critically, combined hormonal contraceptives are generally contraindicated for women who experience migraines with aura, due to an increased risk of stroke. If you have migraines with visual disturbances, speak with your doctor before starting or continuing combined methods.
Progestin-only methods (mini pill, hormonal IUD, implant) do not carry this contraindication and are generally safer for women with migraines.
Blood Clot Risk and Progestin Type
Combined hormonal contraceptives increase the risk of venous thromboembolism (blood clots). This risk varies significantly by progestin type — an important detail that many prescribers do not discuss. Older progestins (levonorgestrel) carry a lower clot risk than newer progestins (desogestrel, gestodene, drospirenone, cyproterone acetate). The patch and ring are associated with higher estrogen exposure than equivalent-dose pills, also increasing clot risk slightly.
This risk is still relatively small in absolute terms for healthy young women without additional risk factors (smoking, obesity, personal or family history of clots, prolonged immobility). But it is worth understanding your personal risk profile, especially if you are flying long-haul, having surgery, or have a family history of clotting disorders.
Thyroid and Adrenal Effects
The combined pill increases thyroxine-binding globulin (TBG), which binds thyroid hormones and reduces the amount of free, active thyroid hormone available to cells. This can create hypothyroid-like symptoms — fatigue, weight gain, cognitive fog — even if thyroid tests look "normal" because total thyroid hormone levels may appear adequate while free levels are reduced.
The birth control pill also disrupts the HPA axis. Research has documented that women on the pill have blunted cortisol stress responses — a pattern more typical of trauma survivors than healthy young women. This dysregulation of the stress response contributes to fatigue, anxiety, and adrenal dysfunction that can persist after stopping the pill.
What to Do About Side Effects
If you are experiencing side effects and want to stay on your current method:
- Supplement with a quality B-complex (look for methylated forms: methylfolate instead of folic acid, methylcobalamin instead of cyanocobalamin)
- Prioritize magnesium glycinate 200–400 mg nightly
- Add zinc 15–25 mg daily and vitamin C 500–1,000 mg daily
- Support the gut with probiotic-rich foods and a quality multi-strain probiotic supplement
- Support the liver with cruciferous vegetables, fiber, and reducing alcohol
